‘We will forever miss them’: Community rallies around victims after fatal Highway 15 crash near Lombardy

Update: Lanarkist has learned that six-year-old Dallas Primeau tragically passed away in hospital on the evening of Monday, June 1.
A GoFundMe campaign has been launched to support a family local to the area following a fatal collision on Highway 15 near Lombardy that claimed the life of a husband and father and left a young child fighting for his life. The child eventually succumbed to his injuries several days later.
OPP continue to investigate the three-vehicle collision, which occurred at approximately 5 p.m. on Friday, May 29, between County Road 1 and County Road 5 near Otter Lake Park.
Police say the crash involved two passenger vehicles and an ambulance that was returning to base with no patients onboard.
The driver of one of the vehicles, whom OPP have said was a 34-year-old man from Elizabethtown-Kitley, was pronounced dead at the scene. A young child travelling in the same vehicle was airlifted to a trauma centre by Ornge air ambulance with life-threatening injuries.
A GoFundMe campaign has been organized by Tammy Moore, a family member of the victims.
In an email exchange with Brockvilleist, a family member who asked not to be named identified the deceased as Dan Primeau and the child as Dallas.
According to her, the Primeaus had recently moved to Kingston from Elizabethtown-Kitley, but the deceased’s official information had not yet been updated.
“Today should be a day of celebration as we remember Dallas on what would have been his 6th birthday. Instead, Jess is carrying the weight of heartbreaking loss,” Moore wrote on the fundraising page.
“In a tragic car accident near Lombardy [on] Friday, May 29, 2026, Jessica lost both her beloved husband Dan and her precious son Dallas is fighting for his life currently at CHEO…They also lost their loyal dog, Lucas, a faithful companion who brought comfort during difficult times. Dan was the sole provider for his family.”

The family member who asked not to be named told Brockvilleist that the father and son “were on their way to Smiths Falls for a vet appointment with their dog Lucas.”
The fundraiser states that Jessica is now caring for the couple’s six-week-old daughter, Blake, while coping with the aftermath of the crash.
“No family should have to endure so much pain and loss.
“As Jess navigates this unimaginable journey of grief, we have the opportunity to come together and support her. Donations will help ease financial burdens and provide stability as she focuses on healing and honouring the memories of those she loved so deeply.”
The family member told Brockvilleist that “Dan was the kindest man you’d ever meet, would take his shirt off his back for anyone and always there to lend a helping hand. He was an amazing father to Dallas and now baby Blake.”
She described Dallas as “a spunky little guy who was so funny ! He could light up a room, even on the darkest days!
“Lucas (the dog) was Jessica’s best friend, he got her through some super tough times over the years.
“We will forever miss them.”
As of Sunday evening, the GoFundMe had raised just over $4,000 of its $13,000 goal.
The driver and passenger of the ambulance involved in Friday’s collision were transported to hospital with serious injuries. In an update issued Saturday, Leeds Grenville Paramedic Service said both paramedics were treated at The Ottawa Hospital’s Civic Campus and have since been released.
The driver and sole occupant of the third vehicle sustained minor injuries and was taken to hospital.
Leeds Thousand Islands Fire Service, Leeds Grenville Paramedic Service and OPP Technical Collision Investigators and Reconstructionists assisted at the scene.
The investigation remains ongoing. Anyone with information or dashcam footage is asked to contact Leeds County OPP at 1-888-310-1122 or Crime Stoppers anonymously at 1-800-222-8477.
